Round Cell Liposarcoma
- Poorly differentiated myxoid liposarcoma
Microscopy:
- lipoblasts, fat cells and atypical cells with eosinophilic cytoplasm
- t(12;16), TLS-CHOP gene rearrangement
Myxoid Liposarcoma
Myxoid Liposarcoma
Clinical presentation and genetics:
- young adults, thigh
- t(12;16), TLS-CHOP gene rearrangement
Microscopy:
- hypocellular myxoid with spindle cells, lymphangioma-like fluid, chicken-wire vascularity
Myxoma
Myxoma
Location:
- Intramuscular, cardiac, cutaneous
Microscopy:
- Bland, stellate spindle cells in myxoid matrix
Malignant Fibrous Histiocytoma
Malignant Fibrous Histiocytoma
Malignant Fibrous Histiocytoma (MFH)
Location:
- proximal extremities, retroperitoneum
Better prognosis:
- superficial, small, prominent chronic inflammatory infiltrate
Gross:
- may be large
Histology:
Immunohistochemistry:
- vimentin, CD68, Factor 13a
Differential diagnosis:
- AFX, DFSP
Variants:
Pleomorphic-Storiform Malignant Fibrous Histiocytoma
- associated post-radiation sarcoma, scars or foreign bodies
- highly pleomorphic tumor cells, storiform pattern,
- chronic lymphoplasmacytic infiltrate, bone or cartilage
Angiomatoid Malignant Fibrous Histiocytoma
- hemorrhagic cyst like spaces and increased chronic inflammation
Inflammatory Malignant Fibrous Histiocytoma
- increased numbers of neutrophils
- phagocytosed neutrophils in tumor cytoplasm
Myxoid Malignant Fibrous Histiocytoma (Myxofibrosarcoma)
- curvilinear vessels with condensation of stroma around vessels
- >50% myxoid
Solitary Fibrous Tumor
Solitary Fibrous Tumor
Histology:
- hypocellular and hypercellular spindle cells arranged in a “patternless pattern”
- keloidal type collagen
- hemangiopericytoma like blood vessels
Immunohistochemistry:
- bcl-2, CD34
Sclerosing Epitheloid Fibrosarcoma
Sclerosing Epitheloid Fibrosarcoma
Clinical presentation:
- lower extremities
Histology:
- cords or nests of epithelioid cells and dense sclerotic stroma
- mitoses
Fibrosarcoma
Fibrosarcoma
Clinical presentation:
- deep tissues of the extremities
Histology:
- very cellular, herringbone, chevron pattern
- overlapping nuclei, increased mitoses, prominent nucleoli
Fibromatosis
Fibromatosis
Classification
Superficial
- plantar
- palmar
- Peyronie’s (penile)
Deep
Desmoid tumors
- abdominal
- intra-abdominal wall
- extra-adominal wall
Histology:
- bland fibroblast cells with variable amounts of collagen
- few blood vessels
- no hemosiderin
Key points:
- need to sample desmoid tumors widely to rule out fibrosarcoma
Myositis Ossificans
Myositis Ossificans
Clinical presentation:
- areas of rubbing (butt, elbows, shoulder), associated with trauma
Gross appearance:
- tan mass with gritty periphery and soft glistening centre
Histology
Zonation effect:
1. central zone with myxoid loose tissue and fibroblasts
2. then → unmineralized woven bone with osteoblasts
3. then → periphery with cortical lamellar bone
Differential diagnosis:
- paraosteal fibrosarcoma (has opposite direction of maturation and less orderly)
Proliferative Fasciitis
Proliferative Fasciitis
Proliferative Myositis
Histology:
- like nodular fasciitis but has ganglion-like cells
