    IMPORTANT NOTE: The following information is intended to supplement, not substitute for, the expertise and judgment of your physician, pharmacist or other healthcare professional. LEPIRUDIN - INJECTABLE (leh-PEER-oo-din) COMMON BRAND NAME(S): Refludan USES: Lepirudin is a "blood thinner" (anticoagulant) used to treat patients who have developed a reaction to another type of anticoagulant (patients with heparin-induced thrombocytopenia). It is also used along with antiplatelet therapy (e.g., aspirin) to treat worsening angina/chest pain and certain types of heart attacks (Acute Coronary Syndrome-ACS). HOW TO USE: Follow all instructions for proper mixing, dilution with the correct IV fluids, and monitoring of treatment (e.g., aPTT monitoring). If you have any questions regarding the use of this medication, consult your pharmacist Give the initial bolus dose of this medication by vein (IV) over 15 to 20 seconds followed by a continuous IV infusion mixed in a saline (NS) or dextrose-based (D5W) solution as directed by your doctor. The dosage and length of treatment is based on your weight, medical condition, and response to therapy. Before using, check this product visually for particles or discoloration. If either is present, do not use the liquid. Learn how to store and discard needles and medical supplies safely. Consult your pharmacist. SIDE EFFECTS: Injection site reactions (e.g., redness, discomfort) may occur. If any of these effects persist or worsen, notify your doctor. Tell your doctor immediately if any of these serious side effects occur: easy bruising or bleeding, pink or red-colored urine, black or bloody stool, coffee-ground vomit, one-sided weakness, vision problems, nausea/vomiting, confusion, slurred speech. Tell your doctor immediately if any of these unlikely but serious side effects occur: fever, yellowing eyes or skin, dark urine. An allergic reaction to this drug is unlikely, but seek immediate medical attention if it occurs. Symptoms of an allergic reaction include: rash, itching, swelling, dizziness, trouble breathing. If you notice other effects not listed above, contact your doctor or pharmacist. PRECAUTIONS: Tell your doctor your medical history, especially of: kidney or liver problems, recent surgery/major medical procedures or biopsies, bleeding problems (e.g., clotting difficulty; any stomach, eye, brain, or lung bleeding), stroke, heart infections (bacterial endocarditis), high blood pressure (hypertension), any allergies (especially to hirudins). Caution is advised when using this drug in the elderly because they may be more sensitive to the effects of the drug. This medication should be used only when clearly needed during pregnancy. Discuss the risks and benefits with your doctor. It is not known whether this drug passes into breast milk. Consult your doctor before breast-feeding. DRUG INTERACTIONS: Tell your doctor of all prescription and nonprescription medication you may use, especially: other "blood- thinning" drugs (e.g., aspirin, warfarin, or thrombolytics such as tPA or streptokinase) Do not start or stop any medicine without doctor or pharmacist approval. OVERDOSE: If overdose is suspected, contact your local poison control center or emergency room immediately. Symptoms of overdose may include: severe bleeding. NOTES: Do not share this medication with others. Laboratory and/or medical tests (e.g., PTT) may be performed to monitor your progress. MISSED DOSE: If you miss a dose or your dose is interrupted, contact your doctor immediately to establish a new dosing schedule. STORAGE: Store the unmixed powder/vials at room temperature or in a refrigerator between 36 and 77 degrees F (2 and 25 degrees C). Once mixed, the medication should be used immediately and is stable throughout the time of the infusion (24 hours). Discard any unused portion of the vial(s).
